Player Waiver

Complete and sign your First State Flag Football waiver before participating at Lums Pond State Park in Bear, Delaware. Players under 18 must have a parent or legal guardian complete the minor section.

Waiver Summary

This waiver applies to participants in First State Flag Football games, practices, warmups, and league events held at Lums Pond State Park.

  • Players must enter their current age.
  • If a player is under 18, a parent or legal guardian must also complete the minor section and sign electronically.
  • If a player will not turn 18 during the 2026 calendar year, the page will mark the player as ineligible for this adult league.
  • You understand and accept the risks of participating in adult flag football.
  • You release the league, referees, staff, volunteers, sponsors, and venue-related parties from liability to the fullest extent allowed by law.
  • You consent to emergency medical care if needed and accept responsibility for resulting costs.
  • You allow league photos and video to be used for promotional purposes.
  • Your electronic signature is intended to be legally binding.

Participant Waiver and Release

I understand that participation in First State Flag Football involves inherent risks, including but not limited to collisions, falls, strains, sprains, fractures, heat-related illness, concussions, permanent disability, and death.

I voluntarily assume all risks of participation and certify that I am physically able to participate in league activities. I release and hold harmless First State Flag Football, its owners, commissioners, staff, officials, volunteers, sponsors, affiliates, and venue-related parties including the State of Delaware, DNREC, and Lums Pond State Park, to the fullest extent allowed by law, from any claims arising out of my participation, except where prohibited by law.

I authorize emergency medical treatment if needed and understand that I am responsible for related costs. I also consent to photographs and video of league activities being used for league promotion without compensation.

By signing below, I confirm that I have read and understood this waiver. If I am 18 or older, I am signing for myself. If I am under 18, my parent or legal guardian must also sign below, and players who will not turn 18 during the 2026 calendar year are not eligible for this adult league.
